The diaphragm is a large muscle separating the chest cavity from the abdominal cavity, with a natural opening through which the oesophagus passes to reach the stomach. In hiatal hernia, this opening widens so part of the stomach pushes upward through it into the chest. This change in anatomy weakens the natural valve that prevents the return of acidic stomach content to the oesophagus, so reflux occurs. The hernia's size ranges from very small causing no symptoms, to large causing bothersome and persistent symptoms.
